KEY POINTS OF HEART ISSUES: WHAT TO DO IN AN EMERGENCY
Fortunately, true emergencies are rarely observed in patients with heart disease, but you must recognize the warning signs if they do happen. The most serious of these emergencies is a sudden severe heart rhythm disturbance called ventricular tachycardia or ventricular fibrillation, which causes a very rapid and sometimes disorganized heart beating, leading to loss of effective heart pumping. This can cause sudden severe dizziness, or loss of consciousness. If the heart pump is very severely affected, a cardiac arrest can occur, during which the heart does not pump blood at all. This is a life-threatening emergency; CPR (cardiopulmonary resuscitation) needs to be started right away, and an ambulance should be called immediately. For this problem, only advanced care from paramedics or other personnel, usually requiring an electrical shock to the heart (defibrillation), can restore proper heart beating. The important signs are severe dizziness, collapse and loss of consciousness, especially if they last more than a few seconds. Time is of the essence.
Severe shortness of breath, often accompanied by a dry cough and weakness, can mean acute congestive heart failure. In this condition, blood backs up “into the lungs” from a poorly beating heart. If a person is severely short of breath at rest, especially if he or she has difficulty lying flat or is coughing severely, an ambulance needs to be called. This condition is treatable, and patients do much better if it is caught early.
Many patients with heart disease will frequently have chest discomfort or tightness that lasts only a few minutes at a time, is brought on by physical or emotional stress and is relieved by rest. Sometimes, these sensations are more severe and do not go away quickly. If chest pain or tightness lasts more than thirty minutes, particularly if it is accompanied by sweating, shortness of breath and weakness, the patient might have unstable angina or a heart attack (myocardial infarction). In this case, medical help is needed right away, preferably from a hospital emergency department.
