MUSCLE CONTRACTION (TENSION) HEADACHES: THE MUSCLE CONTRACTION (TENSION) HEADACHE PROFILE
The Muscle Contraction (Tension) Headache Profile below is a description of the symptoms of muscle contraction (tension) headaches. It is followed by a series of questions that a physician might ask you if muscle contraction headaches were suspected as the diagnosis.
Warning: This exercise is not a means of diagnosing your headaches. The exercise is provided only to encourage you to carefully consider the features of your own headaches using a characterization of muscle contraction headaches for comparison. An accurate diagnosis of your headache problem requires a thorough medical evaluation by a trained professional. There is no acceptable substitute.
Profile-You have frequent headaches that are often present when you awake in the morning but may begin during the day. Usually they are not associated with visual symptoms or warning signs. They frequently hurt you in the forehead and the back of the head and in the neck. You often feel as though a tight vise is squeezing your head and neck. The pain is not usually severe, as it can be in migraine. Heat applied to the back of the neck may make your headaches improve somewhat. Massage of your neck muscles seems to be helpful. Sleep is usually not disturbed by your attacks. Your headaches may last for days without any relief except when you sleep. You may be a nervous person or experience intense emotions, like anger and frustration. Perhaps you are depressed and tearful much of the time. You prefer to keep these feelings inside you. You may have arthritis of your neck or have suffered a previous injury to it. You take analgesics daily.
If this brief and general description of many muscle contraction headaches is similar to your headaches, you may want to answer the following questions. The questions are typical of some of those that would be asked of you if your physician suspected a diagnosis of muscle contraction headaches.
True False
1. Your headaches frequently begin soon after awakening in the morning but do not usually awaken you from sleep during the night.
2. Your neck and shoulders frequently hurt as part of your headaches.
3. Your headaches most commonly involve both sides of your neck and/or head.
4. Nausea and vomiting or distinct visual abnormalities are not typically present during your attacks.
5. Weakness, numbness, tingling, or other “neurological abnormalities” are not part of your headaches.
6. Warmth applied to your neck and head often brings you some relief.
7. Your headaches frequently feel as though a vise or handcuffs are squeezing your head.
8. Between your attacks you are in otherwise normal health.
9. You are a nervous person and feel angry at many people and events but cannot seem to express yourself openly.
10. Relaxing can often relieve your headaches.
11. Maintaining certain postures of your neck and head for long periods, such as when typing, driving, or reading, can bring on a headache.
12. Your headaches frequently last all day or until you go to sleep, only to return upon awaking in the morning.
13. You often find yourself trying to “beat the clock” to finish a day’s work and get things done.
14. If you place a pillow under your neck so that your neck is bent backward slightly, your
headaches seem to lessen.
15. When you have a headache, you prefer to be active and occupy yourself rather than to lie quietly.
16. You sometimes feel “knots” of tightened muscle in the back of your head or neck during a headache.
17. You often feel depressed.
18. Generally, your pain is only mildly to moderately intense, rather than severe.
