FAT-SOLUBLE VITAMINS: VITAMIN ?
Vitamin ? is also known as the anti-hemorrhagic vitamin. Menadione and phylloquinone are compounds with vitamin ? activity.
Functions
Vitamin ? is needed for the formation of prothrombin, a substance necessary for blood clotting.
Intestinal bacteria normally synthesize a substantial amount of vitamin K. Because vitamin ? is fat soluble, absorption is facilitated in the presence of bile. Small amounts of vitamin ? are stored in the liver, heart, skin, muscles, and kidney.
Meeting daily needs
The requirement for vitamin ? is not known. Green leaves are an excellent source of the vitamin. Cereals, fruits, and non-leafy vegetables are rather poor sources.
Clinical problems
Because of intestinal synthesis there is little likelihood of vitamin ? deficiency in normal individuals. Some newborn infants have a tendency to hemorrhage because the intestinal bacteria are not yet sufficiently developed for synthesis of the vitamin. Hence, vitamin ? preparations are sometimes given to the infant.
Hemorrhage can also result if there is interference with the synthesis of vitamin ? or with its absorption. Oral sulfa drugs and antibiotics are known to reduce bacterial growth in the intestine so that there is less synthesis of the vitamin. In diseases of malabsorption, hemorrhage can be a complicating factor since the absorption of vitamin ? is reduced.
Dicumarol is a vitamin ? antagonist. It counteracts the effect of vitamin ? in the formation of prothrombin and thus prevents blood clotting. It has been effective in treating circulatory diseases in which blood clots tend to form and that might endanger the patient’s life. Patients who are given dicumarol must be monitored closely to avoid excessive bleeding.
